Re-work the changelog
Rewrite the changelog to use consistent formatting and structure.
Different releases would have different section headers
(e.g. Added / Changed) rather than the newer headers
(Added / Fixed / Changed / Misc.). Convert all of the old releases to
use the newer, 4-section style.
Additionally, anchor commits for all list items, and put all of the
commit anchors at the bottom of the document. Anchors unique to a
release are now at the bottom of that release.
